ITEMS NEEDED TO FINALIZE RENTAL AGREEMENT
All Documentation completed and submitted into the LCC Office. (CERTIFICATE OF INSURANCE (COI); SECURITY DEPOSIT; SIGNED RENTAL AGREEMENT)
All rental fees MUST be paid in advance.
A certificate of liability is required. Certificate of Liability Insurance of $1 million in coverage w/ Vendor Name and Address as: Free Methodist Church of Modesto dba Life Connection Church, 1520 Rose Ave, Modesto, CA 95355
Organization documents proving that you are a legal and binding representative, (if applicable.)
Life Connection Church required all individuals who will be hosting events involving minors, persons with disabilities, and/or other events involving sensitive persons/groups, to undergo a background check as requested by California State Law. Background checks must be no more than 2 years old. Life Connection Church can run a background check for a fee of $35.00.
PAYMENT FOR USE OF FACILITIES
A minimum refundable security deposit of $100 (or $250-CZ) is required to secure an event date.

If providing a check for the security deposit, to avoid rewrites, PLEASE DO NOT DATE THE SECURITY CHECK, (When booked more than 6 months in advance.) SECURITY DEPOSIT MUST BE RECIEVED IN THE (LCC) OFFICE BEFORE APPROVAL OF EVENT CAN BE FINALIZED.
All deposits are refundable, if cancellation is made more than 48 hours from the event's scheduled date. If for any reason, an event must be canceled within 48 hours of the scheduled event, the deposit will be retained. The church office will hold all deposits until such time as all invoices have cleared and/or the Administrator or Facility Use Committee indicates that the facilities/grounds have been left in clean and good condition. The facility has been vacated on agreed time. Unless exempt from all fees, a deposit will be required at time of rental request.
Rental fees and additional charges will be determined from the Schedule of Fees at the time the request is approved and activated.
Fees shall be paid to the LCC office in advance on the date the intended use of facilities are requested, unless other arrangements have been made.
PAYMENT IN FULL IS DUE; BY CHECK- 3 WEEKS PRIOR TO EVENT DATE, BY CASH- 1 WEEK PRIOR TO EVENT DATE, PayPal CAN BE INVOICED WITH AN ADDITIONAL $3.00 FEE.
(LCC) Schedule of Fees is subject to change at the discretion of the Facility Use Committee.
Facilities Supervisor(s) will be scheduled to work only the number of hours indicated on the approved request. The requestor will be invoiced for any additional hours, including overtime, if required.

and PAYABLE TO the same name.
Failure to make timely payments may result in denial of future requests. Recurring rentals are required to pay for previously reserved dates whether the facilities are used or not.
Acceptance of Terms. I HAVE read the agreement in its entirety, and understand that I am fully responsible for adhering to all sections within the (LCC) Facility Use Rental Agreement.
